Dear Parents:

The following information pertains to the Extended Care Program for the 2010-2011 school year. If you will be utilizing the Extended Care Program, please complete the registration form printed on the back of this letter. Please make sure to fill out the registration form completely and return to the school office by August 17, 2010. If you need additional extended care forms, please down-load forms from our school website.

The fees and hours for Extended Care are as follows:

Extended Care program hours are from 2:30 PM-5:30 PM and 11:45 PM-5:30 Pm on Early Dismissal.

Monthly Rates are as follows:

One Child $125.00/month

Two Children $160.00/month

Three Children $185.00/month

Daily & Early Dismissal Rates are as follows:

One Child $10.00/day

Two Children $15.00/day

Three Children $20.00/day

Fees are due in the office on the 1^st business day of each month. (First day of school for the month of August.) Please make check payable to St. Vincent Ferrer School.

*Snack will be provided

* A study period is provided for grades 5-8 in the Media Center and K-4 in Mercy Hall (Cafeteria).

*There is a late fee of $1.00 per minute per child after 5:30 PM

Children not picked up by 3:00PM from car line are escorted to the Extended Day Program, and the necessary fees are charged. This is done for the safety of our students.

*If your child is being picked up by anyone other than the people listed on the registration form, the request must be in writing. No phone calls will be accepted unless there is an emergency.

The established school rules are in effect during Extended Day program hours, including no cell phone use permitted by students in extended care.
